The Partnership’s current technology initiatives:


Home Telehealth Monitoring program (HTM)


HTM is the fastest growing form of telemedicine whereby a small monitoring unit is installed in the patient’s home. The patient takes their vital signs (blood pressure, weight, pulse, blood glucose) daily using the monitor and sends the readings to the health care provider. The provider is then able to monitor the patient’s vital signs daily and intervenes when they are outside of the patient’s target range.





Clinical VideoTeleconference (VTC)


This AFHCP initiative provides an opportunity for partner facilities to use video-teleconferencing (VTC) as a solution to both clinical and administration requirements, such as expanding access in patient care, allowing consultation with doctors or specialists remotely, and saving travel costs for patients and/or meeting participation. The Partnership coordinated the purchase and deployment of VTC equipment and training. Participating facilities report usage and cost savings to AFHCP quarterly, allowing capture of return on investment and lessons learned.
